Oversees county-wide Department of Medical Assistance Services (DMAS) screening programs and provides guidance and support for staff. Reviews and tracks all Medicaid pre-admission to ensure compliance with state and local guidelines. Collects and analyzes data DMAS screenings to identify trends and patterns for quality improvement, predicts demand for services and monitor s progress in meeting performance objectives. Provides quality assurance review and support for County-owned assisted living facilities. Utilizes LTC program expertise in community outreach and education and assists in marketing efforts to promote continuum of care. Provides supervision to LTC field PHN II positions, and clinical consultation for Department of Family Services (DFS) PHNs assigned to the Care Network Program and other Department of Family Services staff as necessary.

Employment Standards

MINIMUM QUALIFICATIONS:
Graduation from a college or university accredited by the National League for Nursing or the Commission on Collegiate Nursing Education with a bachelor's degree in nursing; plus three years of post-licensure experience as a nurse in an equivalent health care setting whose focus was primarily prevention services and community education, including two years in a public health/community health setting; plus at least one year experience in a public health/community health specialty area or supervisory responsibilities.

CERTIFICATES AND LICENSES REQUIRED:
Possession of current license or a multistate licensure privilege to practice as a Registered Nurse in the Commonwealth of Virginia;
Selected positions or area(s) may require additional certifications (e.g., CPR, STD) to practice specialized Nursing skills;
Selected positions or area(s) of assignment may require the incumbent to possess a valid motor vehicle driver's license.

PREFERRED QUALIFICATIONS:
Preferred candidate demonstrates skills necessary for doing basic and advanced health care assessments. Demonstrates ability to provide quality assurance reviews for health care services. Ability to problem solve and work collaboratively with other agencies and organizations to provide services to the community. Experience with older adults or those with disabilities. Supervisory experience a plus.

NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background investigation, a Child Protective Services (CPS) check, a driving record check, a sanction screening to the satisfaction of the employer. Tuberculosis test.

PHYSICAL REQUIREMENTS:
Employee is required to walk, stand, sit, bend, kneel, reach, climb stairs and carry equipment or young children up to 25 pounds in weight. Uses hands to grasp, handle or feel. Visual acuity is required to read data on a computer monitor. Have the ability to operate keyboard driven equipment. Able to carry equipment and assist with client transfers from sitting to standing and lift up to 30 pounds. Ability to drive a vehicle. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. All duties performed with or without reasonable accommodations.
