Respect, dignity, integrity, and self actualization through a multitude of traditional and adjunctive therapies while simultaneously documenting and disseminating information and reports to referral sources and coordinating with community resources to ensure aftercare and supportive services are available for successful transitioning and discharge planning.

Qualified candidates will possess:
• Master of Social Work from college or university accredited by the Council of Social Work Education.
• Active Maryland Social Worker license required, LCSW-C strongly preferred.
• Active American Heart Association Basic Life Support (BLS) certification required.
• 2-3 years of current discharge planning experience within a hospital case management environment preferred.

Work Schedule:

prn, float SW/CM, may require some weekend and holidays
