The Care Coordinator is responsible for conducting coordinator service activities in accordance with VOA Chesapeake's operation as a Mental Health Rehabilitation Service. In this role, the Care Coordinator provide one on one, community based services, integrating physical and behavioral health services. Approximately 75% of the time, the Care Coordinator will be engaged in activities out in the community and/or the consumers' natural setting. The candidate must have prior experience working with children and the adolescent population. The candidate must be able to coordinate meetings with all of the stake holders involved with the child and adolescent. "#CB" Successful candidates must possess a BA in related field. Most importantly, the successful candidate must be able to develop effective working relationships with the consumer population, be self-motivated and able to motivate others, be able to relate to individuals making significant life decisions, possess strong organizational skills, and have strong oral and written communication skills. The individual must also be computer proficient. Bilingual speaking required. Required Experience Training or experience in a health care, human service field or equivalent.
