Optima Health Community Care is hiring a Registered Nurse (RN) Team Coordinator to lead our team of dynamic care coordinators and conduct face to face assessments with our valued members and coordinate their care as a part of our DMAS contract. This territory consists of the Central Virginia region.

Department/Position Overview:

This essential position requires:

  • Registered Nurse (RN Licensure) and a minimum Bachelors Level Degree in Nursing.
  • Working in support of the entire region with emphasis in Richmond and the surrounding region including the Northern Neck area.
  • Working primarily in the Richmond office Monday-Friday with some travel in the field when needed.
  • Supervising both office-based and field-based employees.
  • Mandatory training for one week in Hampton Roads area of Virginia.
  • Minimum 3 years of case management experience.
  • Previous supervisory experience.
  • Some overnight travel required for ongoing trainings and team-leading duties when necessary.

For Integrated Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM or RN-BC). BLS (if in a clinical setting).
