Adventist HealthCare seeks to hire a Community Health Worker (CHW) who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing. The Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community resources. These resources include access to medical and/or behavioral health providers, medication, food, transportation, housing, and insurance. In collaboration with the Transitional Care RN, the Community Health Worker guides patients through their plan of care.

Requirements

• Bachelor of Science in health education, health promotion, or related field
• 1 - 2 years' experience working in a community based setting with knowledge of local resources and system navigation
• Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance
• Empathic toward individuals complex health needs and their families
• Strong communication and customer service skills
• Knowledge of community resources
• Strong writing skills
• Strong organizational skills; able to develop workflows to create more efficient processes
• Ability to work in a fast paced environment while managing multiple priorities and maintaining professionalism
• Excellent time management skills
• Ability to problem solve and exercise good judgment

Work Schedule:
