We are seeking a full-time experienced Licensed Clinical Social Worker for the Heart Transplant program at the Sentara Heart Hospital. The Heart Hospital is located on the campus of Sentara Norfolk General Hospital in Norfolk, Virginia.

  • What you will do:
  • We are open Monday – Friday between 8am and 4:30pm. Weekend coverage for transplant.
  • Works with VAD and heart transplant patients and families. Will provide support to the renal living donor program.
  • Candidate will have 2 years of adult behavioral experience. Have an understanding of using different tools of psychosocial assessment.

    The Licensed Clinical Social Worker participates as an integral part of a multidisciplinary team to establish comprehensive and integrated plans for each patient. Involvement entails both delivery of clinical services and referrals to appropriate community resources and other professionals. Assists patients and their families in meeting psychosocial needs which predispose illness or interfere with achieving an optimal level of care. Assesses the social, psychological, cultural, environmental and financial situations, as well as disposition needs, for each assigned patient.
