The Care Manager's mission is to reduce readmissions by improving transitions of care by collaborating with the patients, providers, and staff to facilitate patient access to healthcare and social services that enhance the patient's ability to adhere to the prescribed medical treatment regimen. The Care Manager works as part of a multidisciplinary team to identify patients at increased risk for readmission and provide care coordination services in the outpatient setting by working with providers and partners as well as patients and patient's family.
Build relationships with clinicians providing patent care
Work with Providers, Staff and Patients to insure positive patient outcomes
Travel to Provider offices and /or Patient's homes to evaluate treatment regimens
Current Maryland Registered Nurse license
Bachelor's degree in nursing
Two years of experience in acute hospital or rehabilitation setting preferred
Must be licensed and able to operate a motor vehicle as well as maintain automobile insurance and abide by requirements listed in AHC's Automobile/Fleet Management policy
Ability to interact with all levels of the health care team
Strong customer service skills
Good verbal and written communication skills
