Job Details

Provides intensive community-based case management services to older adults and adults with disabilities. Assesses functional eligibility for Medicaid long term care services. Conducts assessments and monitors the provision of in-home services. Assesses risks posed to client and conducts self-neglect investigations. Requires assessment, care planning and coordination skills, the ability to manage competing priorities, and to respond effectively to crisis situations.

Note: This job announcement will be used to fill vacancies in the Reston - Cameron Glen Adult Services' office in Fairfax County.

Employment Standards MINIMUM QUALIFICATIONS:
Graduation from an accredited four-year college or university with a bachelor's degree, plus two years of professional social work experience. A master's degree in a related field may be substituted for one year of the required experience.

CERTIFICATES AND LICENSES REQUIRED:
Driver's license - required upon hire.

NECESSARY SPECIAL REQUIREMENTS:
The appointee to the position must satisfactorily complete a criminal background check, Child Protective Services Registry check, and driving record check to the satisfaction of the employer. Driver's license or ability to efficiently access other means of transportation in order to visit clients in their homes and to meet with community service providers. Some positions may require oral and written bilingual competency in English and a second language.

PREFERRED QUALIFICATIONS:
Master's degree in social work (MSW). One year of professional social work experience working with older adults or adults with disabilities. Bilingual in English and Spanish, Farsi, Korean, Chinese, or Hindi is helpful. Working knowledge of community-based and long-term care. Good oral and written communication skills. Good organizational skills. Experience using automated technology and software programs such as Microsoft Word, Excel, Access, and Outlook.

PHYSICAL REQUIREMENTS:
Ability to: conduct home visits; communicate with clients; and lift up to 10 pounds. All duties performed with or without reasonable accommodations.
