The Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community resources. These resources include access to medical and/or behavioral health providers, medication, food, transportation, housing, and insurance. In collaboration with the Transitional Care RN, the Community Health Worker guides patients through their plan of care.

Job Responsibilities:

•In collaboration with the Transitional Care RN, coordinate patient care with patient, family, and ancillary/community services to promote quality and cost-efficient care for assigned caseload. Completes required documentation in a timely and accurate manner
•Assist patients with enrollment and/or referring them to various health and social service programs. May include meeting patient in community to assist with connection when needed
•Assist patients to schedule medical appointments to include connection with transportation resources
•Provide follow-up to patients, by phone, home visits and visits to other settings where patients can be found
•Conduct home visit to ensure that the patient has what is required to recover safely at home. Visit to include safety checks (e.g., check for fall risks) and preparation for follow-up appointments with provider

•\tStrong communication and customer service skills
•\tKnowledge of community resources
•\tStrong writing skills
•\tStrong organizational skills; able to develop workflows to create more efficient processes
•\tAbility to work in a fast-paced environment while managing multiple priorities and maintaining professionalism
•\tExcellent time management skills
•\tAbility to problem solve and exercise good judgment


•\tBachelor of Science in health education, health promotion, or related field
•\t1 - 2 years' experience working in a community-based setting with knowledge of local resources and system navigation

Any additional background requirement:

•\tLicensed driver and able to operate a motor vehicle as well as maintain automobile insurance
