A Social Worker provides services to clients in accordance with a plan of care. They assist the physician and other team members in the development of the plan of care, prepares clinical and progress notes, works with families, uses appropriate community resources, participates in discharge planning and in-service programs. The Social Worker acts as a consultant to other agency personnel including both the clinical and financial departments.

Qualified candidates will possess:
•\tMaryland Licensure as a Certified Social Worker (LCSW)
•\tMaster's Degree in Social Work from a school accredited by the Council on Social Work Education
•\tValid American Heart Association CPR Certification
•\tValid Driver's License with agency insurance requirements
•\tThe ability to navigate an Electronic Medical Record system
•\tOne-year experience as a Social Worker in a facility or home care environment

Principle Duties and Responsibilities including Direct Reports:
•\tCoordinates care with other disciplines to provide team approach to patient care
•\tProvides appropriate patient interventions leading to resolution of patient problem
•\tMeets productivity standards set by agency
•\tMeets expectations of patients and family members by providing comprehensive care
•\tUse multicultural interventions to serve a diverse population
