Optima Health Community Care is hiring a Licensed Clinical Social Worker (LCSW) Team Coordinator to lead our team of dynamic care coordinators and support our valued members and coordinate their care as a part of our DMAS contract. This territory consists of the Central Virginia region.

Department/Position Overview:
This essential position requires:
    Licensed Clinical Social Worker (LCSW) with Masters of Social Work Degree. Working remotely in support of the entire region with emphasis in the Farmville/Cumberland/Goochland regionsSupervising field-based care coordinators.Working primarily from home and in the field with occasional work in the Richmond office.Mandatory training for one week in Hampton Roads area of Virginia. Minimum 3 years of case management experience.Previous supervisory experience.Some overnight travel required for ongoing trainings and team-leading duties when necessary.

Responsibilities:

Assumes responsibility, accountability and leadership for the daily operations including coordination of work, quality, and service. First line supervisor in the Department of Managed Care for assigned site/function. Facilitates the work of assigned team members. Provides a leadership role in ongoing case manager competency assessment, needs identification and educational offerings. Provides educational services and clinical oversight to the Managed Care Coordination team. Participates in the work activities of assigned teams and provides care coordination services as needed.
