Optima Health Community Care is hiring a Registered Nurse (RN) Manager to lead our expanding department of Team Coordinators, care coordinators, care coordinator technicians and additional service-providing professionals in our efforts to support and service our valued members and coordinate their care as a part of our DMAS contract.

The Registered Nurse (RN) manager will provide oversight for the entire Central Virginia region and will work out of our Richmond, VA office.

Department/Position Overview:
This essential position requires:
    Registered Nurse (RN Certification) and a minimum Bachelors Level Degree in Nursing, Master's Degree is preferred. Directly supervising up to 4 Team Coordinators.Indirectly supervising up to 40+ employees.Traveling throughout the region when necessary to provide leadership support to department.Assisting with neighboring regions when necessary.Mandatory training for one week in Hampton Roads area of Virginia. Minimum 3 year case management nursing experience required.Minimum of 1 year of leadership required.LTSS experience strongly preferred.

Master's degree preferred. For Integrated Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM or RN-BC).

Responsible for managing and coordinating the patient care transitions program. This ensures the patient's progression through the care continuum in a manner that achieves designed clinical and financial outcomes. Participates in the system development and implementation of integration of care transitions along the continuum of care. Works in close collaboration with the multidisciplinary team, in all settings to include ambulatory, post - acute providers and health plans. Serves as mentor by role modeling a professional practice consistent with organizational goals, customer service, clinical effectiveness, and standards of care. Responsible for developing and managing the departmental budget, compliance to federal, state and regulatory requirements.
