Overview:

Optima Health Community Care is hiring a Housing Services Specialist to work remotely in support and service of the Charlottesville, VA . You will also be required to go into the field to look at housing.

The specialist will be instrumental in assisting members in their transition from nursing facilities. This will require heavy interaction out in the community searching for housing and assisting with the required needs in the home. Extensive experience with community outreach, and housing assistance programs is critical. 1 year minimum experience required, 3 or more years highly desired.

Responsibilities:

This job oversees the housing services and supports for LTSS services and supports (LTSS). The incumbent collaborates with federal, state and local affordable housing agencies such as Housing of Urban Development (HUD), State Housing Authority, subsidized housing, public housing authorities and voucher programs. The incumbent works with individuals and families receiving LTSS in finding affordable, accessible housing through one of the above-mentioned agencies. Assists in educating and assisting the case managers regarding affordable, accessible housing services for LTSS members.
