Optima Health is hiring a Registered Nurse (RN) for the Population Care team in Virginia Beach, VA.


Monday - Friday, 8am-5pm

Department/Position Overview:

As we are expanding our services and focus for population care, we are hiring a dedicated Registered Nurse (RN) with a passion for population interventions and wellness to join our team in this newly created role. In this office-based position, the Population Care Nurse will hold a minimum BSN degree with at least 2 years nursing experience preferably in population health and/or case management.

Implement population interventions to improve health, wellness and quality performance including evidence-based preventive services such as immunization, chronic disease preventive care, cancer screening, medication follow up, and wellness visits. Implement provider-based interventions to improve health, wellness and quality performance including building relationships with provider clinical staff, facilitate the scheduling and delivery of evidence-based healthcare services, obtaining medical documentation to close care gaps.
