Overview:

Optima Health is hiring a Nurse Practitioner (NP)- Manager for our Population Care department in Virginia Beach, VA.

Hours/Shift:

Monday - Friday, 8am-5pm

Department/Position Overview:

As we are expanding our services and focus for population care, we are hiring a dedicated Nurse Practitioner (NP) with a passion for population interventions and wellness to join our team in this newly created role. In this office-based position, the Manager will work heavily on quality improvement, utilization management and risk management while heading multiple projects related to population health as well as practicing and ordering necessary testing kits for screenings and more. Must hold a minimum Master's Degree in Nursing with at least 3 years of Nurse Practitioner experience. Previous experience in primary care required.

Previous experience in primary care as Nurse Practitioner required. Maintain Nurse Practitioner continuing education credits of as prescribed by licensing body maintains prescriptive authority; acquire special clinical competencies as required by Senior Medical Director Participates in quality assurance and utilization management activities and assists with risk management functions.

Responsibilities:

Mgr, Population Care reports to and works closely with the Sr Medical Director to achieve the strategic objectives of the Population Care (PC) Program including implementing evidence-based population interventions, improving member health/wellness, improving clinical performance measures, and reducing total cost of care. Provides clinical services for the operation of the PC programs. Designs and implements clinical programs to achieve PC departmental goals. Ensures that member safety is central to all population care program operations. Collaborates with analytics and IT to ensure the collection and analysis of all data necessary for the operation and evaluation of PC programs. Ensure that provider and member communication are maintained for optimal performance. Collaborates with internal and external stakeholders to ensure compliance with policies, procedures, and legal/regulatory requirements. Analyses plan quality and financial data to identify areas of opportunity. Builds strategic relationships with internal and external stakeholders including providers to promote and achieve PC goals. Evaluates and identifies external vendors to support the achievement of PC goals and manages the ongoing relationships with those vendors including ensuring that performance meets contractual requirements.
