The Behavioral Health Transitional Care Coordinator collaborates with the patient, physician, case manager, community health workers, and other healthcare providers to facilitate patient access to healthcare and social services that enhance the patient's health and well-being and his/her ability to adhere to the prescribed medical treatment regimen in the most efficient and cost-effective manner possible. This role works specifically with high and rising-risk behavioral health patients to reduce avoidable utilization, ensure they have access to primary and specialty care, and access to social resources needed to self-manage their health.

Job Responsibilities

1. Identify high and rising risk behavioral health patients who receive care at Adventist Healthcare (e.g., visited the ED or had an observation or inpatient stay).

2. Assess, plan, and coordinate healthcare services to meet the patient's specific health care and psycho-social needs in a cost-effective manner.

3. Assess need for home visit at initiation of care plan. Home visits may include but are not limited to, focus on medication reconciliation, disease education for patient and family, home safety checks, in-home discussions about care needs and challenges, social needs assessment, and preparation for follow-up appointments with provider.

4. Connect patient to community provider(s) and resources for behavioral health care.
Ensure weekly contact with patient (in person or via telephone) to progress towards care plan goals.

5.Ensure timely documentation regarding patient enrollment in Behavioral Health Care Management program, patient progress, and graduation from program.

Qualified candidates will possess:

1.\tActive Maryland RN or Social Work licensure
2.\tBachelor of Science in nursing, Social Work or RN enrolled in Bachelor of Science in nursing program with plans to complete within two years
3.\t3-5 years of nursing or social work experience (behavioral health experience
4.\tpreferred)
5.\tLicensed driver and able to operate a motor vehicle as well as maintain automobile insurance
