Qualified candidates will possess:
•\tBachelor's degree from an accredited program
•\tRegistered Nurse license in the state of Maryland
•\tCurrent American Heart Association BLS/CPR certification
•\tAt least 3 years of recent case management experience in a rehab setting

PRN: Day shift, 8:00 am - 4:30 pm
Meets with patients and family to coordinate all aspects of care and arranges weekly team conferences. Works with the interdisciplinary team to discuss therapy goals, discharge plans, and other-centered issues. Communicates with insurance case managers requesting additional days based on recommendations from the team. Assures a seamless transition to rehabilitation and reentry into the community.
