Purpose of Position
The Care Transition Assistant starts/oversees the initial referral process for Home Health utilizing a variety of resources and systems. The CTA oversees tracking of Hospitalized patients to other facilities capturing resumptions of care and new referrals upon discharge. The CTA will lead the face to face compliance process. Provides report assistance to management.

Principle Duties and Responsibilities including Direct Reports:
1.\tReceives New Referrals and monitors Worldview throughout the day.
2.\tMonitors, electronic discharge systems for new referrals,
3.\tGathers all relevant clinical documents
4.\tIndex documents using Electronic Medical Record (EMR) systems (i.e. Worldview to HCHB)
5.\tRespond to emails within 30 minutes from Community Liaisons regarding new referrals
6.\tLeads and oversees the tracking and compliance of Face to Face.
7.\tCompletes Hospitalized patients notes 1-2 times a day and send to appropriate Care Transition Coordinator (CTC).
8.\tEnters Hospitalized Patient notes in the EMR system for Clinical Liaison on a daily basis.
9.\tCovers for the Care Transition Coordinator, Face to Face staff when necessary.
10.\tAnswers phones for the department.
11.\tAssist Director Care Transition with reports, schedules and projects.
12.\tMaintaining facility assignments and physicians as needed.
13.\tOther duties as assigned.
Work Schedule:

Required Knowledge, Skills, Abilities, and Other Characteristics:

Knowledge of health care terminology preferred Microsoft Office Suite (Outlook, Word, Excel) skills required High Proficiency in entering data in real time Strong Interpersonal Skills Strong ability to work with others Flexibility Ability to work well under pressure Clear English communication required

Education and Experience Required:

High School Diploma required AA Degree in Business or Health Care preferred. 1-2 Home Care Agency experience preferred.
