The Discharge Planner will collaborate with the interdisciplinary team for Adolescent Partial Hospitalization and Intensive Outpatient Program to provide the most effective discharge plan for each individual client.

High school or GED
Bachelors degree in psychology, social work, or related field preferred.
Graduate of an accredited school
2 years of experience in behavioral health
Valid BLS CPR certification preferred.
Work Schedule:

8:30am - 5 pm
