Job Description As a Physical Therapist you will be responsible for the following: Identification of needs and delivery of services to residents who require physical therapy intervention in accordance with all Federal, State and facility requirements. Collaborates with facility staff to promote the highest practicable physical, mental, and psychosocial well being for the residents. Screens, evaluates, and develops physical therapy programs for implementation for all residents, as needed. Provides clinical supervision to Physical Therapy students, LPTAs, and Rehab Technicians in accordance with State guidelines. Develops effective physical therapy treatment plans and goals to meet each residents needs, and produce meaningful functional outcomes. Completes documentation, functional outcome, and billing in an accurate, timely, and legible manner, consistent with published regulations and standards for insurance compliance and reimbursement. Ensures MD orders are obtained according to facility and/or company policy Demonstrates effective oral and written communication skills Demonstrates effective problem solving skills Complies with APTA code of ethics, State Practice Act for Physical Therapy, and HIPAA guidelines Obtains Continuing Education Units (CEU s) required for maintenance of State licensure to practice. Completes renewal process for State licensure timely to avoid expiration and submits copies of renewed license to Rehab Director and HR. Requirements Minimum Qualifications: Education: Graduate of an accredited Physical Therapy program and maintains current licensure as a Physical Therapist in Virginia or is a recent graduate and actively working toward licensure. Experience: Successful completion of required clinical affiliations, preferably including at least one rotation in the SNF setting.
