Job Description: Sentara CarePlex Hospital in Hampton, Virginia, is hiring a full time Medical Social Worker to support the Emergency Department.

Position Overview11:00 a.m. - 7:00 p.m. Monday - FridayProvision of medical social work services to patients receiving care in the hospital setting. Assessment of the social, psychological, cultural, environmental, and financial situations, as well as, disposition needs, for each referred patient. Collaborating with patients, families, healthcare team members, and community agencies to develop and implement plans to address identified needs. Evaluating effectiveness of plans and initiates change as needed. Using crisis intervention, problem solving model, community organization, and advocacy skills in identifying needs and resources in the hospital and community.One (1) year of Social Work experience beyond internship hours is required.Facility HighlightsSentara CarePlex Hospital is a 224-bed, acute care hospital, and Certified Primary Stroke Center located in Hampton, Virginia. Other
