Patient Care Coordinator
Job Description We are growing! Comprehensive Health Center (FQHC) in Northern Virginia seeks an energetic and motivated Full-Time Patient Care Coordinator to join our dynamic team! This is a clinical administrative and patient customer service role in a complete health care environment which includes primary medical, dental, behavioral health, and nutrition care. **Our mission is to provide excellent and affordable medical, dental and behavioral health services by being compassionate, culturally competent, and accessible to all. ** Patient Care Coordinator Essential Responsibilities Include: Schedules patient appointments in coordination with provider and nurse. Schedules and tracks specialty and lab referrals as ordered by the provider. Answers and dispatches provider line calls as per Healthworks provider line protocol. Arranges transportation for patients as needed according to established process. Responsible for distributing, scanning and collecting incoming and outgoing mail and records for assigned providers. Scans all patient related paper documents into patient electronic medical records daily. Empties the fax inbox and attach documents to patient electronic medical records daily. Verifies that all patients' medical records are received according to the terms specified in the Medical Record Release Protocol. Obtains any records that are needed from other medical offices, hospitals, or specialists. Patient Care Coordinator Qualifications: High School Diploma or GED required. Associates Degree preferred. Two (2) years of progressive work experience in customer relations and office administration required, preferably in a health care setting. Exemplary customer service and cultural competence skills required. Medical appointment scheduling, knowledge of medical terminology and experience with electronic medical record (EMR) required. Public health or community health experience is a plus. Must enjoy working both autonomously and in collaboration with a team. Proficient computer and EMR skills required. Due to patient population, bilingual English/Spanish skills required.
