Harmony Square Clinical Coordinator-Memory Care Coordinator
Job Description The Harmony Square Clinical Coordinator is responsible for the overall management of the day to day operations on the unit. The Clinical Coordinator serves as a supervisor for PCA/CNA/Medication Aides and ensures that Harmony Square residents receive consistent and compassionate care. The Clinical Coordinator is also responsible for completing ICP's for Harmony Square residents, as well as promoting and participating in meaningful programming throughout the day. Must have Current RN, LPN, or LVN licensed to practice nursing in Virginia, with license in good standing with the Board of Nursing Responsibilities include but are not limited to: Supervise PCA's/CNA's/Med Aides: Coach/mentor to ensure Harmony Square team is performing at a high level Oversee effective execution of the medication management plan for Harmony Square residents Continuous assessment of resident needs with updates to appropriate individuals (Healthcare Coordinator and Executive Director) as it relates to care and changes in resident condition Implement measures to assure resident wellness and safety Completes ICP's for Harmony Square residents Promote a culture of teamwork and unified goals for staff Respond to emergencies calmly and competently Report incidents to Health Care Coordinator in a timely manner Be present and assist with the delivery of meals in the dining room, monitoring resident acceptance of diets and assuring adequate nutrition Maintain high standards with regard to record keeping, resident documentation Establish and maintain a positive working relationship with all departments Identify risk factors for residents and initiate interventions Provide a thorough report of resident status to oncoming nursing staff at each change of shift Adhere to all company policies and procedures and attend in-services and staff meetings Practice proper body mechanics and safe resident transferring techniques Adhere to attendance policies; report to work on time and prepared to begin duties Assist new residents and their families in the transition to life on Harmony Square Effectively communicate with Harmony Square families by responding to questions/concerns promptly Understand and be compliant with DHEC regulatory requirements as they apply to Harmony Square Complete all other assigned duties Requirements: Current RN, LPN, or LVN licensed to practice nursing in Virginia, with license in good standing with the Board of Nursing Must have one year experience assessing residents with cognitive impairment and safety. Must have a passion for seniors with Dementia / Alzheimer's Experience working with senior population, preferably in assisted living or LTC Must be a team player who works well with others and has experience supervising staff Must possess critical thinking skills, ability to effectively communicate verbally and in writing Demonstrated ability to manage time efficiently with strong organizational skills Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs Must be willing to work a flexible schedule to include weekends and holidays EOE Company Description The Crossings at Spring Hill will be a state-of-the-art senior living community, providing a great lifestyle and the best in Assisted Living and Memory Care.
