Licensed Practical Nurse (LPN)
Job Description The LPN provides quality nursing care and services to residents in a respectful and dignified manner, ensuring the delivery of all needed services. The LPN coordinates physical, social, and emotional aspects of assisted living care. Responsibilities include but are not limited to: Execution of the medication management plan Continuous assessment of resident needs with updates to appropriate individuals as it relates to care and changes in condition Implement measures to assure resident wellness and safety Work as part of a team to meet resident needs Respond to emergencies timely and in a calm and competent manner Report incidents and changes in resident condition to Health Care Coordinator Be an active part of all activities including community events Be present and assist with the delivery of meals in the dining room, monitoring resident acceptance of diets and assuring adequate nutrition Maintain high standards with regard to record keeping, resident documentation Establish and maintain a positive working relationship with all departments Identify risk factors for residents and initiate interventions Provide a thorough report of resident status to oncoming nursing staff at each change of shift Provide motivation and direction to CNA/CMT's Attend in-services and staff meetings Adhere to and implement all policies and procedures Practice proper body mechanics and safe resident transferring techniques Adhere to attendance policies; report to work on time and prepared to begin duties Complete all other assigned duties Requirements: Current LPN or LVN license to practice nursing in Commonwealth of Virginia, with license in good standing with Board of Nursing Experience working with senior population, preferably in assisted living or LTC Must be a team player who works well with others and has experience supervising staff Must possess critical thinking skills, ability to effectively communicate verbally and in writing Demonstrated ability to manage time efficiently with strong organizational skills Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs. Must be willing to work part time including weekends and some holidays!
