The Discharge Planner will collaborate with the interdisciplinary team for Adolescent Partial Hospitalization and Intensive Outpatient Program to provide the most effective discharge plan for each individual client.
Case management with families, schools, and providers
Attend treatment team regularly
Runs a weekly community resource/ discharge planning group
High school or GED required, Bachelors degree in psychology, social work or related field preferred.
Two years of experience in behavioral health.
Valid CPR card preferred.
Monday - Friday 8:30 am - 5 pm
