Job Description: Sentara Healthcare is currently recruiting for an experienced Pharmacy Technician to work as a Transition Specialist in support of pharmacy operations at our Sentara Leigh Hospital in Norfolk, VA. This is a Full-time (40hrs/wk) Rotating Shift (Days, Evenings & Nights) position.
Sentara Leigh Hospital is a Magnet Status, 250-bed facility featuring all patient-friendly private rooms and specializing in orthopedic, gynecological and urological services. Sentara Leigh offers a contemporary care environment for general medical/surgical and ambulatory services and is home to the orthopedic hospital, which has 48 beds dedicated to specialized orthopedic care. The hospital offers a full-service Emergency Room with a dedicated observation unit, Critical Care, GI, and Oncology services. It also encompasses a Family Maternity Center.

Responsible for logistically organizing, collaborating, and/or delivering required discharge medication, self care products, and post acute care services to patients and/or their family/caregiver. Also responsible for engaging the patient more actively in his/her own self-care, ensuring they are fully educated, engaged, and equipped to be successful in their self care at home.

Education Level
High School Grad or Equivalent

Experience
Required: Pharmacy Technician - 1 year

Required: Cert Pharm Tech Board, Cert Pharmacy Tech
