Ensures that patients receive the right level of care at the right time and enhance the patient experience and their functional outcomes, through effective and efficient care coordination across the continuum of care. Supports the Post Acute Care Coordinator(s) and the PACC process. Responsible for checking the patients post acute care benefit coverage with insurance providers, collecting and maintaining accurate data regarding patients plan of care destinations and communicating pertinent information to the Post Acute Care team as needed to support the coordination of care. Responsible for ensuring all pre-surgical requirements are completed per leadership direction. Qualifications EDUCATION: High School or GED equivalent; AA or BS/BA preferred. EXPERIENCE: 3 years related healthcare experience. LICENSE/CERT/REG: No special certification, registration or license required. SKILLS: Ability to read and write in English, perform arithmetic calculations and communicate with patients, their relatives, and professionals effectively. Incumbents in position must be detail-oriented and able to multitask and prioritize work to facilitate efficient and timely care planning and coordination of care. Ability to type 30-40 wpm. Working knowledge of ; MS Office Suite, spreadsheet software, and electronic medical record systems. Ability to prioritize tasks while maintaining service oriented, courteous interactions with internal and external customers. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in various formats (eg written, verbal, diagram, etc.) Primary Duties and Responsibilities Assists with pre-operative joint replacement class to include preparation of materials, coordination of class logistics, and demonstrating a working knowledge of class content. Collaborates with patients, PACC, and Post Acute Care providers to ensure timely transition of care to next level: to include obtaining necessary orders from medical team, completion and submission of transfer paperwork for all levels of care, and scheduling of appointments and transportation. Collaborates with physician offices, pre-operative testing department, patients, and external providers to ensure a complete pre-surgical packet is available for review. Continually seeks to identify and seize opportunities to tap talents, or to increase personal knowledge and skills needed for the position through continuing education programs and/or networking. Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations. Creates and files patient charts, paper and electronic. Demonstrates a professional conduct and ethical standards practice. Displays general working knowledge of Durable Medical Equipment commonly used for PACC population,and assists patients in obtaining. Identifies PACC patients from surgical schedules and maintains accurate master lists. Obtains, documents, and interprets information about PACC patients post acute care benefits from insurance providers, via web and/or phone. Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate. Performs other duties as assigned.
