Rehab Care Manager

Position Details: Provides clinical oversight and episode management for approximately 75 patients receiving direct care from the therapists on the team. Maintains a caseload of patients, meeting expectations described in the Physical Therapist job description; performs admissions, revisits, re-certifications and discharges Coordinates episode management with Operations Directors Responsible for clinical outcomes and utilization management Ensures therapists: enter frequencies, schedule visits, enter goals and orders the day of patient admission; develop appropriate point of contact; receive timely authorization for visits; demonstrate competent clinical care; document care thoroughly; schedule complete visits as ordered through the plan of care; and submit all work according to policy Qualifications EDUCATION: Bachelor's degree in Physical Therapy, MPT (Master of Physical Therapy), or DPT (Doctor of Physical Therapy). EXPERIENCE: 1 year experience in a certified home care or home hospice program. LICENSE/CERT/REG: License to practice Physical Therapy in all jurisdictions in which the employee provides services to patients, the District of Columbia, Maryland or Virginia. CPR certification required. Valid driver's license in good standing and dependable vehicle for transportation. SKILLS: Ability to case manage. Organization skills. Excellent verbal and written communication skills. Basic computer skills preferred. Knowledge of home equipment, home care reimbursement, and regulations governing PTA's, LPN's and HHA's. Primary Duties and Responsibilities Acts as coverage to support operations in absence of Operations Director. Assists with patient complaint resolution. Communicates with patients, care givers, and interdisciplinary staff to resolve actual or potential issues with the patient. Conducts chart reviews and assures that admissions, recerts, and discharges in the patient documentation is accurate and that proper utilization and clinical needs are being achieved. Contributes to the achievement of established goals and objectives established from Medstar management team. Adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance and other mandatory training. Complies with governmental and accreditation regulations. Lead preceptor responsible for training and providing feedback to new associates, doing peer to peer reviews and being available as a clinical resource for field staff. Maintains a caseload of patients and meets the expectation of care described in the corresponding clinical job description. As deemed necessary, the role of Care Manager also performs admissions (as appropriate), revisits, recerts, discharge and joint visits to insure quality patient care delivery and excellence in patient experience. Maintains effective working relationships with other departments. Participates in multidisciplinary quality and service improvement teams, meetings and on committees representing the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations. Manages care of patients to maximize patient care services and maximize patient experience. Participates in Mock surveys as directed. Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate. Performs Daily Workflow duties as designated per team. Performs other duties as assigned. Provides initial review for 485 plan of cares consistently across the agency and provide feedback as needed on post review status at the discretion of the Operations Director. Provides Operations Director with input regarding team members' work performance. Provides oversight of staffing and scheduling concerns. Provides weekly case conference to orientees during IPR and continues on a bi-weekly basis after the IPR period. Responsible for clinical outcomes and utilization management. Serves as Clinical resource to patient, caregiver, physician. Serves as educator in the discipline, providing feedback and suggestions to improve workflow, best practices and standard procedures.
