"Make a Difference - Change Lives" The Coordinating Center is recruiting a Service Coordinator passionate about connecting people experiencing or at-risk of homelessness with community resources to ensure individuals have the tools they need for long-term stability and success. Qualified candidates will receive more information, an HR phone screen interview followed by an onsite interview with the Team Manager. Upon completion of the new coworker and program specific orientation in our Millersville office this position will transition to working remotely from their home office, with a focus in Montgomery County. The Housing Initiative Program (HIP) Service Coordinator provides coordination of services and direct assistance to formerly homeless individuals to promote long term housing stability in a permanent supported housing program. The HIP Service Coordinator supports the mission of The Coordinating Center by providing person centered services and service coordination within the parameters of the contract. Service Coordinators are responsible for complying with the Montgomery County Housing Initiative Program contract requirements, integrating technology to maintain database integrity, report writing and maintenance of documentations. The HIP Service Coordinator provides ongoing outreach to program recipients, works interdependently with the HIP Community Health Nurse and HIP Housing Locator and Adult Community Services (ACS) staff and collaborates with community agencies to engage recipients in activities which ensure residential stability. Essential Responsibilities: Secures information such as medical, psychological and social factors to develop Service Agreements and Plans of Service. Obtains documentation to assist recipient with enrollment and reapplication/ redetermination process. Advocates to obtain services and benefits for individuals on the program. Provides good tenant training and skills coaching. Ensures all recipients apply to receive Housing and Urban Development Housing Choice Voucher and Public Housing benefits. Provides information and referrals to community and government programs for emergency needs and other needs identified by recipients. Promotes positive health behaviors to support optimal health and wellbeing. Refers recipients to medical care, mental health counseling and other services as indicated. Provides face to face and ongoing telephone contact with recipient, family members and service providers to assist with the coordination of services and maintenance of residential stability. Accesses and records client, residential and community resource information and care coordination activity in various databases. Develops and maintains an active network of community resources and alternative funding sources. Meets The Centers and HIP contractual billing and documentation guidelines. Manages multiple priorities and tasks with flexibility to enhances service coordination services. Mentors and trains new HIP Service Coordinators. Qualifications: Bachelors degree from four-year college or university in social work or health/human services or related field. Two years related experience providing human services and/or training and working knowledge of social service and individual support services, Medicaid services, State and Montgomery County community resources. Proof of current automobile insurance is required. Ability to communicate effectively with individuals with varied cognitive abilities to establish professional relationships. Ability to read and interpret documents such as employment paperwork, leases, assessment reports or medical records, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or co-workers. Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Excellent organizational skills and follow through Knowledge of landlord/tenant rights and responsibilities helpful.
