Job Description: Facilitates and promotes continuous quality improvements in the delivery of patient care by maintaining active interfaces between internal and external customers (patients, family, visitors, medical staff, risk management, etc.), maintaining data system of customer issues for use in identifying areas for improvement. Monitors and audits Advocate team complaints and grievances for accuracy, adherence to established processes and communicates and guides Advocate team members as needed daily. Prepares reports for bi monthly grievance meeting and leads discussion. The Patient Advocate serves as a Sentara direct representative to patients/families that have concerns or questions regarding Sentara services.
