ASSOC INTEGRATED CARE MGR - Obici Hospital
Sentara Obici Hospital located in Suffolk, Virginia, is seeking a full time Associate Integrated Care Manager in our Congestive Heart Failure program.
Responsible for facilitating discharge plans in collaboration with Care Coordination RN and/or Social Worker in the hospital setting. Problem solves and troubleshoots any barriers to the patient's discharge. Validates accuracy of all discharge information for purposes of transition, record keeping and billing. Functions under the supervision of a BSN RN or Licensed Independent Practioner (LIP/LCSW). Demonstrates professional and clinical knowledge relative to designated clinical practice setting for patient care, adhering to the level of verified competency dictated by the site specific program. The AICM works as part of a multi-disciplinary team to support and communicate effectively with other licensed independent practitioners (Physicians, NP. PA) in collaboration while providing care for patients, clinical support and teaching for patient and family members, and adheres to the legal and ethical standards of nursing practice. Maintain all certifications and licensure relevant to clinical practice setting. Managers responsibilities of workload with accuracy, provides excellence in customer service, and the implementation of interventions for transition planning. Serves as a patient advocate.
Bachelor's Level Degree - SOCIAL WORK OR
Diploma - NURSING OR
Trade School Graduate - NURSING
Required: Acute Care - 2 years
Preferred: Case Management - 2 years
Required: Basic Life Support
Preferred: Licensed Practical Nurse, Registered Nurse
Required: Communication, Critical Thinking, Service Orientation
Preferred: None, unless noted in the “Other” section below
Trade/Tech School Graduate in Nursing or Bachelor's in Social Work. LPN or RN license required for those in respective fields. If hired as an RN, New Hire Education Requirement: All new RN's who do not have their BSN will be required to sign a BSN Agreement committing to enrollment in an accredited RN to BSN program within 24 months of hire and successfully obtaining their BSN within 5 years of hire.
