Community Resource Coordinator/Adol. Psych
Purpose of the position:
The incumbent will be an active member of the child and adolescent multi-disciplinary treatment team serving as a liaison between the inpatient hospital setting and the patient*s school and community. Work closely with parents, guardians, the treatment team and the child or adolescents school in order to ensure a seamless re-integration into the school setting. Possesses knowledge and understanding in navigating special education services including, assisting with coordinating IEP meetings, 504 plans, school transitions and advocating on behalf of patients for appropriate support services.
Collaborate with the Psychiatrist, Social Worker and Nursing Staff to determine appropriate Treatment Team recommendations
Meet with and speak with parents, guardians and patients
Contact with school counselors, resource teachers and school representatives in order to facilitate school re-integration, discuss team recommendations, facilitate increased support services and gather collateral information
Document team recommendations and provide to school representatives
Document all coordination in CERNER EMR
Coordinate Home and Hospital teaching when indicated and supervise online home and hospital teaching when required
Plan and facilitate child and adolescent patient education groups supporting Life Skills, Vocational skills and school re-integration
Bachelor*s Degree in Education, Social Work, Psychology or other field with relevant experience.
Must have a minimum of 2 years of experience in care coordination, vocational rehab experience, transitional age youth experience or education/ special education setting.
Must have experience working with children and adolescents with Psychiatric diagnoses, their families and school representatives.
CPR American Heart Association
Must have excellent oral and written communication skills. Must be extremely well organized and able to prioritize responsibilities and manage time with minimal supervision. Must be proficient in Microsoft Office. EMR experience preferred.
12 noon - 8 pm Monday - Friday.
Work every other weekend with time off during the week.
