Responsibilities

Our Care Transition Coordinator/Community Liaison is responsible for ensuring the Agency achieves its referral and admissions goals by developing and maintaining referral relations and securing admissions to the home health agency. Calling on hospitals, continuing care facilities such as SNF's and ALF's and physician offices will be required.

The Care Transition Coordinator is responsible for ensuring the Agency achieves its referral and admissions goals by developing and maintaining referral relationships with CM/SW/ Physicians and securing admissions or re-admissions to the home care agency; conducting clinical assessments of potential patients and interacting with CM, SW, Physicians and patients and families to provide education on home health services and insurance criteria; participating in referral source discharge planning meetings as appropriate; education to our referral sources, participate in Health Fairs as needed, participates in Strategic Planning meetings, tracking weekly performance and productivity, and monitoring and analyzing referral trends while upholding the values and customer services standards of the organization. Also, supports and backs-up other CTC's as necessary to ensure agency goals are achieved.

Healthcare sales and/or marketing experience required.

Qualifications

Maryland Licensure LPN or RN is plus.

Healthcare sales or marketingexperience required.
