In accordance with National Lutheran s We CARE philosophy, the Guiding Principle Core Values, and in alignment with the NLCS culture, the Assisted Living Care Coach is responsible and accountable for the following:CORE COMPETENCIESReporting to the Health Services Director, the Assisted Living Care Coach s primary responsibility is to manage the clinical oversight of the resident care in the assisted living program. As well as to provide coaching, leadership and supervision for Assisted Living Nursing Services Team Members.Takes the lead in coaching, supervising, directing and coordinating the assisted living nursing service program to ensure excellence in the delivery of care for residentsManaging the clinical oversight of resident care in assisted living and memory support, ensures acuity on assigned units is appropriate for the residents and issues nursing / clinical orders based on needs of residentReview Wellness Nurse level of care assessmentsReport to the resident s physician, the OHCQ, and the resident, if applicable, the legal representative of the resident when the Assisted Living nursing care team fails to implement nursing or clinical orders without identifying alternatives to the care of service orderCompletion of the Maryland Assisted Living 45-Day Review for all assisted living/memory support residents, Assisted Living Manager assessments, and Level of Care Scoring ToolCompletion of the 14 Day Medication Review, new resident assessmentsReview/update resident assessments and new resident service plans annually, or more often as changes in condition occurProvide staff training related to each resident s care planOversee the assisted living medication administration programComplete Incident Reports and notify Executive Director, families, and physician, when appropriateUpdates to Health Care Practitioners Resident Physical Assessment (ie, return from hospitalization, change in condition, annually)Creates and sustains an environment that treats each NLCS resident with dignity and respectOversees the planning and implementation of a holistic social and recreational program that addresses the social, spiritual, emotional and physical needs of the residentsDevelops and manages Assisted Living Services operating and capital budgetsEnsures that team members are assigned responsibilities consistent with their education, experience and ability. Supports, assists and evaluates staff nurses in the management of their unitsEnsures resident records are maintained for each residentCollaborates with interdisciplinary team, identifies ways to improve care of residents, and make recommendations as appropriateCoordinates the transfer of residents to more appropriate care settings, as neededEnsures residents families, physicians and others as appropriate are kept informed of changes in resident conditions and service needsProvides ongoing coaching, supervisory, performance management, training and leadership for their teamBuild trust and accountability by frequently holding coaching conversations with team members to develop, mentor, and provide constructive feedback to support team members goals and strategiesConduct required stay interviews for all new hiresPractices regular Coaching Conversations with direct reports to build up and equipRun and manage various reports from HRIS / payroll systems to maintain compliance with employee licensures, payroll, online learning system, attendance, etc.Verifies care activity documentation to ensure the delivery of nursing care according to physician s orders, care plans, established standards and facility policies. Ensures medication administration is as ordered in accordance with nursing standards and NLCS procedures and policiesDirectly verifies and documents status of residents with change in conditions, new admissions, accidents and incidents, and validates continuity of care against the 24-hour report and dashboard documentation in (PCC)Reviews and Updates Infection Control Logs, Wound Logs, Lab Logs and other system process for continuity of care and per policies and procedures. Follows up with nurses and nursing assistants who fail to meet standardsCoordination and completion of individualized plan of careDirects resident care plan conferencesPerforms other duties as assignedJOB QUALIFICATIONSRegistered Nurse licensed by State of assignment (MD or VA) in good standing in state of community serving, and has completed the mandatory delegating nurse training course developed by the Board of NursingA four year college degreeTwo (2) years experience in a health care related field and 1 year of experience as an assisted living program manager or alternate assisted living manager; or Two (2) years experience in a health care related field and successful completion of the 80 hour assisted living manager training programSuccessfully complete training and achieved state certifications required to operate an assisted living communityDesignated by licensee to oversee day-to-day operation of assisted living program. A four year college degreeFree from TB in the communicable form. Immune to measles, mumps, rubella and varicella through vaccinationKnowledge, experience and proficiency with COMAR, Office of Health Care Quality and CMS guidelines a plusKnowledge with Nursing Assistant and Licensed/Registered Nurse Scope of PracticeProven, positive leadership skills essentialComputer electronic medical record experience is requiredDemonstrated ability to work effectively with a diverse group of stakeholdersAbility to partner, communi cate, coach and counsel direct report team membersMust be proficient in Microsoft Office
