Catholic Charities Diocese of Wilmington Catholic Charities directs and coordinates the charitable and social service programs of the Catholic Diocese of Wilmington. It delivers critical direct care human services to individuals and families residing in Delaware and the Eastern Shore of Maryland regardless of their religion, race or ability to pay. Serves as a member of a treatment team providing mental health services to children, youth and their families. Assists family in completing intake application/ coordinates intake process. Coordinates community resources as indicated in the treatment plan in conjunction with the therapist. Conducts home assessments to obtain information on the child (client's) living environment. Meets with the client family to provide support and home management advice and to monitor assigned tasks, the acquisition of needed social services, and social/environment issues impinging on the family in conjunction with the therapist. Prepares for and co-facilitates child and youth therapy groups. Conducts children and youth group activities during parent meetings. Spends 65% of time in direct service with children and families. Provides client transportation in accord with approved IOP car utilization procedures. Completes and submits progress notes and incident reports on the same day activity occurred. Bachelor's Degree or equivalent with a minimum of two years experience working with children. Demonstrates flexibility of schedule to meet program demands. Maintains client confidentiality at all times. Exhibits good communication skills. Possesses emotional maturity and be objective. Demonstrates awareness of cross-cultural issues and deal with a cultural diverse at-risk population. Ability to work independently or as part of the treatment team. Ability to work closely with psychiatrists, psychologists, social workers and school counselors. Maintains a valid driver's license.
