Essential Functions: Provide relevant information, as requested, concerning individual's values, interests, needs and progress. Assist individuals in maintaining a clean, neat and safe environment; this includes but is not limited to, assuring that the residence will pass safety and environmental inspections. This will include laundry, house cleaning, dishes, and other environmental duties Assures the House Manager is provided with a current telephone number by which the Residential Counselor can be reached expediently Provides each client with the individualized instruction, support, direction, supervision, demonstration and follow-up needed by the individual to acquire appropriate functional life skills and to integrate successfully in the community. This includes, but is not limited to, assuring participation in residential and community based leisure and social activities, accessing community amenities, encouraging communication with family and friends, encouraging and assisting individuals in developing and utilizing effective self-determination and self-advocacy skills and encouraging, instructing and supporting individuals in developing the functional life skills and assuming the responsibilities required for successful independent living Assists individuals with any transitions such as Emergency room visits, residential moves, etc. Demonstrates knowledge of assigned individuals' goals, Goal Plans, Supplemental Procedures, Behavior Support Plans and Nursing Plans of Care and assures they are implemented as written; maintains data as required/ specified in clients' Individual Plans Insures the daily proactive schedule is visible to individuals and assures the schedule is followed by individuals Assures individuals participate in all required/scheduled meetings, appointments and daily activities Conducts Just Community Meetings twice each week and completes written documentation of these meetings Attends meetings as scheduled or as directed by the House Manager or his/her designee Immediately reports incidents according to Agency policy for Incident Reporting and Investigation Procedures. Is available to participate in the investigation of incidents as requested Attends and participates in all in-service training as scheduled Provides individuals with consistent supervision and interaction. This includes, but is not limited to, staying on shift until officially relieved and positively communicating with individuals Maintains current certification in Medication Administration Performs other duties as directed by the Residential Director or his/her designee Competencies: The ability to interact positively with individuals, co-workers, doctors, community residents, employers, family, advocates and Agency staff and the ability to advocate for our individuals in a professional manner. SKILLS & ABILITIES Education: High School Diploma or GED. Experience: 6 months experience working with developmentally disabled individuals is preferred but not required Computer Skills: Basic Computer Literacy Certificates & Licenses: Certified Medication Technician Valid Driver's License No More than 2 points on Driving record Other Requirements: This position requires the successful completion of all DDA and Agency required trainings
