Who We Are: Community Options, Inc. is a Non-Profit company which develops residential and employment supports for people with disabilities, utilizing technology and training. We recognize that people with disabilities need environments, equipment, clinical and staff support that are tailored to their very specific needs. The Community Integration Counselor provides individualized services designed to assist participants in more effectively managing and overcoming the difficulties and stresses confronted by an individual with a developmental disability living in the community. If interested, click apply now or send resume to [Click Here to Email Your Resume] What You Will Be Doing: Supports and encourages individuals to increase his or her ability to problem solve, be in control of life situations and to be as independent as possible. Meets with waiver participants on a regular basis either in the individual's residence, the community or an office setting. Provides ongoing support for integration efforts. Documents individual's progress toward acquiring community integration. Assists in building a support network between the individual and the community. Ensures medical consultation forms are completed, by the physician, for each medical appointment attended. Coordinates all leisure and volunteer opportunities. What Experience/ Skills You Have: High School Diploma or equivalent. Minimum two years of relevant work experience. Knowledge of community resources. Ability to exercise good judgement and use common sense. Crisis management skills and consistent demonstration of ability to respond to changing situations in a flexible manner, in order to meet current needs. Excellent interpersonal and communication skills. Ability to present facts and recommendations effectively in oral and written form. Understanding and commitment to community based supports for people with disabilities. Ability to establish a comfortable and supportive relationship with a person receiving support. REQUIRED LICENSES AND/OR CERTIFICATES: Valid driver's license with satisfactory driving record. Satisfactory criminal background. First Aid, CPR, OSHA and Medication Certification course within first 90 days of employment, and any required residential or state training.
