BROAD FUNCTION: The Certified Nursing Assistant is a trained professional who has direct care with residents, responsible for direct care and personal needs. The Certified Nursing Assistant has a high school degree with advanced knowledge and demonstrable skills in the areas of environmental, physical, psychosocial and personal care needs of consumers with cognitive disabilities who may be frail because of age or physical condition. The Certified Nursing Assistant works with a team of interdisciplinary staff to carry out treatment plans and activities designed to enhance the therapeutic environment in a residential living arrangement. The Certified Nursing Assistant must complete all Relias Trainings and complete all trainings required by regulation and policy to work in the selected programs. PRINCIPAL DUTIES AND RESPONSIBILITIES: Conducts treatment interventions and activities as assigned on the daily schedule including assisting clients with daily living tasks and social skills, carrying out or assisting with recreational activities, observing and reinforcing behaviors as prescribed by the recovery plans and intervening to resolve conflicts as they arise. Provides personal care assistance to residents in the areas of ADL's, personal grooming, bathing, dressing and undressing, oral hygiene, hair care, shaving, nail care, nutrition and hydration, toileting, housekeeping, assisting with feedings, ambulation assistance and transferring, assisting with resident appointments, documenting and recording intake and outputs, housekeeping and organizing resident's rooms, assistance with self-administration of medication, obtaining vital signs and blood glucose levels, documenting in Carelogic and site logs, reporting and documentation of medical/occupational incidents, communicating with Resident Manager, RN Manager, nurse and staff about resident's behaviors, vital signs, bowel movements, meal consumptions, medical compliance and or refusals and all changes in conditions. Duties as required. Monitors the well-being and location of all clients assigned to the program, completing all required documentation in the shift log. Makes notes to the clinical record as assigned. Ensures the health, safety and well-being of all assigned clients is maintained by adhering to internal health and safety policies and PM 46 regulations including: provision of care which maintains each resident's right to be free from abuse, mistreatment or neglect and reporting of any instances of such to appropriate supervisory staff; adherence to confidentiality requirements; application of the basic principles of infection control; EDUCATION AND EXPERIENCE REQUIREMENTS: Delaware High school diploma or equivalency; Delaware certification as a Certified Nursing Assistant; CPR, First Aid and LLAM Certifications are required. Experience working in a residential setting with consumers with cognitive disabilities and/or chronic physical illness is preferred. Valid Drivers License required
