Vesta Inc is a financially sound non-profit organization that serves clients with a mental health diagnosis. Currently we are seeking to fill a Health Home Care Manager position in our Health Home Program. This position would require travel locally between locations between Lanham and Odenton, Md. This is a licensed (Registered Nurse) professional position requiring general and specific knowledge of physical health, behavioral health, substance abuse and developmental disabilities. The position requires knowledge of the biopyschosocial and interdisciplinary model of Vesta, Inc.'s service delivery. This position is considered PART time, working approximately 32 hours per week. Qualifications: Possession of a bachelor's degree in nursing preferred with relevant work experience which uniquely qualifies the individual for the job. Licensed Registered Nurse in the state of Maryland. Two years positively referenced work experience. Valid driver's license, and automobile insurance and safe driving record. Skillsets: Knowledge of the philosophy, principles, strategies and methodology of nursing as they apply to assisting persons with mental illness, substance abuse disorders, developmental disabilities and somatic health needs. Knowledge of medications, indications, side effects and therapeutic effects of prescribed medications. Ability to administer and monitor medications. General knowledge and experience with skills required for clinical assessment and bio- psychosocial therapies for persons with mental illness, substance abuse disorders, developmental disabilities and somatic medical needs. Knowledge of psychosocial rehabilitation philosophy and concepts for persons with disabilities. Knowledge of current theories of contributing factors in and effects of mental illness, substance abuse disorders, developmental disabilities, and somatic medical needs. Ability to establish and maintain trust relationships with clients that reflect boundaries recognized as the community standard of care and professional ethical behavior. Ability to communicate effectively orally and in writing. Ability to work with others as part of an interdisciplinary team. Ability to effectively assess the functional capabilities mental status and service needs of clients and referrals and employ appropriate intervention in a range of clinical situations. Ability to perform general and specific somatic health assessments, recognize symptoms indicative of disease, and plan and implement appropriate interventions. Ability to implement appropriate interventions for interdisciplinary treatment plans and document services accordingly. Willingness and ability to examine one's own behavior, feelings and attitudes to facilitate a productive relationship with clients and the community. Ability to provide health education to staff & clients. Knowledge of community resources and skill in resource management, linkage, and advocacy. Ability to establish positive peer relationships and to coordinate effectively with the interdisciplinary team to include the full range of Vesta, Inc.'s service delivery system. Willingness and ability to interface effectively with hospital personnel, families and advocates of clients and clients or potential clients. Conversant with electronic health records and data analytics. Perform all other direct, job related duties as assigned.
