Clinical Resource Management Case Manager
The Case Manager is responsible for facilitating the patient's hospitalization from admission through discharge. The Case Manager coordinates with physicians, nurses and social workers and other disciplines within the care team, including outside agencies, to expedite the appropriateness, effectiveness and timeliness of care. Applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning, and manages the resources necessary for cost effective, quality patient care. This position includes meeting the needs and providing services to all age groups-infancy through geriatrics, as well as providing services for the culturally diverse population Washington Hospital serves. Bachelor's degree along with current RN DC license is required. 5 years of broad clinical experience in the acute care setting, with experience in case management, quality management, utilization experience preferred. Ability to establish priorities, meet deadlines and maintain standards of productivity; basic knowledge of computers and clinical applications; ability to problem solve in a proactive, creative manner using sound judgment based on factual information and clinical knowledge; ability to lead and actively participate in multidisciplinary teams; excellent interpersonal skills and communication style; basic knowledge of data analysis; ability to effectively negotiate with internal and external providers of patient care services; ability to form positive, collaborative relationships with hospital staff, patients and families. Current RN license.
