Basic Job Function Summary:  Manages the operational elements of the agency quality improvement program. Responsible for agency performance improvement results and related process changes, communicating practice standards and monitoring client care, clinical services and outcomes. This position provides direction in maintaining agency licensure, accreditation and coverage compliance requirements.  Manages agency's quality functions.

Minimal Qualifications:  

 

§  Registered Nurse/Therapist  with current license from respective State Board.

§  Bachelor's Degree required.

§  Two years' experience in homecare/hospice

§  Must have well developed interpersonal and communication skills to provide effective interface with clients, professional clinical staff and all other related team members.

§  Knowledge of home care ( hospice) quality improvement principles and related data collection and analysis

§  Must possess a valid Driver's License and have reliable transportation to and from the work site.

§  Must be able to articulate and demonstrate a commitment to the mission, vision and values of Trinity Home Health Services and to inspire active support of this in others.

§  Ability to consistently demonstrate commitment to the Business Code of Ethics, and adhere to the Organizational Integrity Program.

§  Must be able to adapt to frequently changing work parameters.

§  Able to use computer programs and telephone system .

§  Ability to produce results and ensure accountability

§  May require some travel.
