What You Will Do:University of Maryland Medical System is currently seeking a Director of Case Management to join our Population Health Department. General Summary The Director of Case Management (CM) will lead and manage clinical program operations including program deployment, enhancements, case management team performance, strategic planning, and is also responsible for patient care coordination and utilization management in the University of Maryland Quality Care Network (UMQCN).The Director provides clinical leadership for all case management programs by serving as an educator, role model, member advocate and change agent to enhance member care and staff competency. The Director directs daily team operations and provides strong leadership through training, coaching, teaching, and managing assigned team members and programs in addition to supervision of case manager supervisors who assist with the management of the team. Principal Responsibilities and TasksProvides leadership and direction to all UMQCN Case Management programs, based on accepted principles of nursing, social work, gerontology, geriatrics and case management practice,Directs staff and monitors department activities to evaluate the productivity and quality of programs and processes in order to identify potential improvements and to ensure maximum performance.Develops, implements and directs departmental projects, workflow processes, policies and procedures in collaboration with internal and external stakeholders Oversees program data and provides strategic direction on impacting outcomes.Evaluate patient care data to ensure that care is provided in accordance with clinical guidelines, organizational standards and identified milestones. Develops and directs patient care delivery systems which assure consistency, quality, and cost effectiveness of patient care services across the continuum of care What You Need to Be Successful: Education and ExperienceBSN required, MSN preferredCurrent, active Maryland RN licensure10+ years of clinical and 5+ Years of managerial experience within care management and coordination of care required.Experience building care management programs including strategic planning, development and execution preferredCertification in case management (CCM) preferredWe are an Equal Opportunity/Affirmative Action employer.
