The Director of Clinical Operations provides clinical oversight to Continuing Care (CC) staff supporting the Director of Nursing and key clinical leadership to ensure appropriate care delivery. In this role, the Director of Clinical Services is responsible for evaluating that CC clinical practice to ensure compliance with state and local regulations, nurse standards of practice and Erickson Living standards. This position is located in Catonsville, MD. 1. Serves as a resource, role model, and mentor for the DON and other key clinical leaders within the Continuing Care (including but not limited to DON, ADON, Wellness Manager, Quality Care Coordinator, MDS Coordinator, SDC, Assisted Living Managers and Memory Care Managers). 2. Assesses and evaluates the effectiveness of processes, systems, and care delivery to residents/guests ensuring to meet Erickson Quality of Care Standards and providing recommendations for improvement as needed. 3. Support and ensure the implementation of Erickson approved clinical programs, practices and policies for Short Rehabilitation /Post Acute Care, Long Term Care, Assisted Living and Memory Care. 4. Ability to provide consultative/corporate support to Ancillary Business Lines (Home Health, Hospice, etc.) to support an integrated approach to care. 5. Identifies the possibility for new, or revision to, policies, procedures, protocols and programs and communicates them to the VP of Clinical Services 6. Participates in the development, implementation and ongoing evaluation of clinical priorities in CC 7. Provides support and guidance for implementation of Memory Care Programing within the Memory Care neighborhood and throughout the CC. 8. Visits each CC is in accordance with defined Erickson standards to review and/complete systems reviews of care delivery and clinical systems to include: o Rounds of each nursing neighborhood to validate implementation and adherence to the Erickson care delivery model o Completes focused guest/resident record reviews o Observes ADL care provided by certified nursing assistant (GNA, care associate, household associate. o Observes treatment and medication administration by nurses and med passes, as needed. o Reviews and validates information such as: | Quality Measure Reports | ADL Splits/documentation | Erickson Benchmarking (infection control, wounds, restraints, incident reports, falls, weights, re-admissions) | Survey POC implementation and monitoring | PIRMS (aka QAPI) minutes, follow up, etc. | Review Incident Report log and Grievance Log | Reviews Medicare Utilization Review minutes/follow up and documentation | Reviews Wound Portal Reports | Reviews unlocked assessment reports (SN, LTC, AL, MC) | Reviews Service Levels | Other reports as needed 9. Supports and guides DON and clinical leadership during regulatory surveys, provides assists with the review of the Plan of Correction, monitors implementation of POC and as needed, the development of the Informal Dispute Resolution. 10. Reviews and forwards written reports of all findings of visits and to include recommendations to the CC Administrator and Director of Nursing. 11. Hosts and facilitates monthly Benchmarking calls to review areas of concern 12. Hosts a peer team call to facilitate continued growth of the ADON, DON, ALM/WM. 13. Serves as a resource to Administrator, DON, and Executive Director for clinical issues. 14. Consults CC Medical Director as necessary. 15. Provides assessment of CC processes for completion of MDS, state documentation and transmission. 16. Advises and assists Director of Nursing to assure compliance to nursing staffing budgets and monitors staffing levels relative to standards of care. 17. Assists in the selection, orientation, and evaluation of Directors of Nursing and provides ongoing support to nursing leadership team. 18. Participates in corporate workgroups and task forces as needed and appropriate. 19. Maintains knowledge of applicable state and federal regulations and Erickson policies and procedures. 20. Maintains knowledge of approved electronic medical records system 21. Perform other duties as required EDUCATION and/or EXPERIENCE: Registered Nurse (RN) Graduate from Accredited School of Nursing BSN or Masters in Nursing preferred Basic computer skills required including experience with Microsoft Office, Internet, web applications, Power Point and Excel. Experience with electronic medical record preferred. Expertise in wound care, quality improvement, MDS, Fall Management, Infection Control and standards of LTC & Assisted Living/Memory Care documentation guidelines preferred. Minimum of five years of experience in skilled nursing Director of Nursing, preferred Experience in managing multiple facilities preferred Experience in both Skilled Nursing/Long Term Care and Assisted living preferred
