Successful service excellence candidates will have the following qualifications and ability to: Establish and implement social services programs that reflect goals/objectives. Assess patient/resident upon admission and periodically to identify social service needs. Work with the interdisciplinary care team to develop the best plan of care that addresses identified needs of patients/residents. Coordinate discharge planning for patient/resident discharges to assure continuity of care. Assist with Resident Council and Family Council. Manage department budget. Provide education regarding resident rights and advanced directives. Provide support and coverage for admissions team as needed. Promote and demonstrate a Service Excellence culture. Perform other duties as assigned within the scope of the position. Qualifications: Bachelor's degree in social work or human services OR one year (within the last five years) of supervised social work experience. Experience in recognizing and assessing the emotional and social needs of patients/residents and a thorough knowledge of community agencies and resources available to meet those needs. Ability to demonstrate excellent customer service, communication and leadership skills. Must be flexible, detail-oriented and be able to make effective decisions and work as part of a service excellence team.
