We are seeking someone with experience in Long Term Care.Ideal candidates will have a 4 year degree in Social Work or related field and Virginia LSW licensure in good standing. Candidates must be able to work in a fast-paced environment, and must have excellent written and verbal communication skills. The LSW position requires flexible scheduling to ensure optimum customer service, attendance at facility events, and coordination with the facility management team.Responsibilities of the LSW include but are not limited to:Provide planning, assessing, coordinating and implementation of services to enhance each resident's social and psychosocial well being, including Discharge Planning.Perform all duties involved in resident advocacy.Participate in all resident care conferences.Complete all social service related documentation.Provide back up when necessary to the Admissions Department.As a CommuniCare employee you will enjoy competitive wages and PTO plans.
