Job Description: Primary responsibility is to function in a support role for the Case Management department. The Placement Specialist provides direct support to the discharge planning process. The Placement Specialist facilitates the exchange of clinical information between the hospital and post-acute care providers, arranges durable medical equipment, and patient transportation to home or facility. Faxes or electronically transfers patient demographic and clinical information to facilities (skilled nursing, long term care, acute rehab, psychiatric, substance abuse, etc.) and to other agencies as requested by Case Manager or Social Worker. Verifies the facility received information. Informs Case Manager or Social Worker of authorizations and requests for additional information. Prepares, completes, and distributes facility transfer packets. Identifies missing documentation and initiates appropriate follow-up to obtain information. Arranges ambulance and wheelchair van transportation for patients to facilities or home as requested by Case Manager or Social Worker. Notifies facility and unit staff of arrangements. Facilitates effective exchange of information with on-site post-acute facility liaisons, home care liaisons and DME suppliers. Answering anticipated discharge time or needs questions as well as assisting in locating information they may need for their determination. This includes notification of the Case Manager or Social Worker of any identified concern or delay in placement or post discharge supplies. Trouble shoots problems to avoid delays ie transfers relative to late transportation, lack of, or omissions in transfer summaries. Notifies Case Manager, Social Worker or bedside nurse to contact physicians to initiate transfer summary/obtain additional information. Serves as a resource to Care Management staff and unit staff in areas of durable medical equipment and transportation regulations. Documents patient choice, ambulance and wheelchair van arrangements and in electronic medical record. Participates in Care Management system and process improvement initiatives. Identify opportunities for improvement and recommend solutions to Care Management Leadership. Demonstrates commitment to work partners to help each other reach mutual goals and learn from each other. Demonstrates actions and behaviors that consistently promote trust, respect, a positive attitude and promote team morale. Required Experience: One year related work experience. Customer service and MS Office and data entry skills. Experience with office machines (copier, fax, telephone) computer terminal/personal computer, software (word processing, spreadsheets etc). Helpful: Prior experience in similar role. Light physical work: exerting up to 20 lbs of force occasionally, and/or up to 10 lbs of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for Sedentary Work and the worker sits most of the time, the job is rated for Light Work.
