Job Description This is a grant funded position Clinical Practice Provides a consultative role in the management of geriatric patients and will participate as a part of interdisciplinary teams providing care to elders Will be actively involved in developing and implementing geriatric best practice protocols and consequent interdisciplinary education Assures patients and families are kept informed of the clinical management care plan and clinical outcomes on a daily basis Collaborate with physician to ensure patient and family needs are met Participates in formal and informal patient teaching activities aimed at promoting positive health practices and an understanding of the individual s medical problems Confers with physicians regarding assigned patients keeps physicians informed as is appropriate to the needs and problems related to patient care Will be available for consultation on the care of complex geriatric patients while in patient and or post discharge via telephone Will develop and help lead best practice education and training opportunities across the multidisciplinary team across the continuum of care Leadership Contributes to knowledge development and improved care of the adult gerontology population Uses scientific knowledge and theoretical foundations to differentiate between normal and abnormal changes in physiological psychological and sociological development and aging Scientific foundation Provides guidance consultation mentorship and educational experiences to students nurses and other health professionals regarding acute and critical care populations Participates in clinical and nursing research projects Mentors and or educates nurses on principles of nursing research Analyze data and recommend areas for nursing or clinical research Serve as mentor and PI for Summer Intern students Quality Implements evidence based practice interventions to promote safety and risk reduction for the older adults with acute critical and complex chronic illness needs Develops templates and clinical pathways based on evidenced based guidelines and literature Develops a system for patient reported outcomes measurement and serves as an expert in the analysis of the data collected Maintains compliance with state federal and JC and other regulatory requirements High competency with data acquisition and analysis and Lean methodologies Technology Synthesizes data from a variety of sources including clinical decision support technology to make clinical decisions regarding appropriate management consultation or referral for acutely and critically ill patients Ethics Participates in inter professional teams to address issues related to triage quality of life and utilization of resources Advocates for the patient s and family s rights regarding health care decision making taking into account ethical and legal standards Required License Certification Licensed as a Certified Registered Nurse Practitioner CRNP by the Maryland Board of Nursing Geriatric certification BLS Required Minimum Experience Three years of experience as an Advanced Practice RN
