Job Description Job Summary: The Certified Medication Technician, under the direction in accordance of the Director and RN supervisor, provides clients with services in accordance with healthcare provider's orders regarding medication administration and minor treatments, per state regulations. In addition, may provide personal care including assistance with activities of daily living following the client's Care Plan. The Support Aide provides service to individuals in their own homes, who need assistance caring for themselves as a result of old age, sickness, or disability. Personal care may include assistance with the activities of daily living and instrumental activities of daily living, companionship, transportation, and/or caregiver respite. Duties/Responsibilities: Assistance with the activities of daily living, including: bathing, mouth care, hair care, nail care, skin care, shaving, dressing, feeding, positioning, transferring, ambulation, exercise, toileting, medication reminding, vital signs and blood pressure. Assistance with instrumental activities of daily living including: food preparation, telephone use, transportation, housekeeping, money handling, taking medications, laundry, and shopping. Provide companionship including social interactions, conversations, emotional reassurance and encouragements of activities that stimulate the mind. Perform/assist with essential shopping errands. Assist clients/patients with following written, special diet plan. Escort clients to medical facilities, errands, shopping and outings as specified in the care plan. Ensure the client's safety and security in the home or on outings. Provide respite care for caregivers in accordance with care plans. Follow written care plan. Report changes in the patient's physical and/or cognitive status, and/or changes in the patient's environment to the nursing supervisor. Complete the flow sheets and time sheets during each service visit, and submit flow sheets and time sheets to the office in accordance with Friends Circle policies and procedures. Participate in team meetings by providing input and making suggestions. Administers prescribed medications to clients in accordance to orders from the Physicians or Nurse Practitioners. Verifies correct identity of client. Verifies medications are correctly identified. Presents medications to client and observes ingestion or other application. Documents medications were taken or any reason why they were not taken. Documentation may be electronic. Performs personal care activities that assist the Client with activities of daily living which may include (but are not limited to): bathing, dressing, grooming, mouth care, shaving, toileting, transferring and meals as indicated on care plan. May receive supply of medications in the Blister card as ordered by physician and delivered by pharmacy. May assist in minor treatments or application of ointments. Report changes in physical or cognitive status and/or changes in the Client's environment to the RN Supervisor. Complete documentation of tasks including medication via Telephony. Qualifications: Complete a CNA training program that meets the requirements established by the State of Maryland Board of Nursing. Be certified as a Nursing Assistant (CNA) or Geriatric Nursing Assistant from the State of Maryland Board of Nursing. Homecare companion experience preferred. Attend Friends Circle orientation, as required. Attend Friends Circle In-Service Training, as required. Have current CPR certification. Must have a valid State of Maryland Driver's License. Meet all hiring requirements prior to start of employment at Friends Circle.
