Responsible for all aspects of new participant admissions and current participants case management, participant plan of care aspects related to the role of social worker, oversight of the Center's transportation program and plays and active role in marketing the Center's services. Provides support group leadership.
Minimum Licensure/Certification Required (if applicable):
Licensed to practice Social Work by the State of Maryland.
Successful completion of “Healthcare Provider Course” in CPR and Multi-Media Standard First Aide course is required prior to employment or be completed within three (3) months of employment.
Minimum Knowledge, Skills, and Abilities Required:
Minimal of a Master of Social Work degree required.
Must have at least one-year experience in a professional capacity providing services to adults in an adult day care center or health care setting.
Must have at least two (2) years experience with seniors and/or adults with physical and mental disabilities.
The ability to travel to various locations within reasonable timeframes. (For this purpose “Responsible timeframes” is defined as the ability to get to and return from work related meetings without using excessive amount of work time for this purpose.)
