Psychiatric Registered Nurse RN-BC
Pathways is a private, nonprofit organization whose mission is to promote the mental, emotional, and physical well-being of the individual, especially the marginalized and vulnerable, through the shared exploration of pathways to healthy living.
We have been awarded a five year Supported Employment Demonstration (SED) grant designed to study the impact of integrated mental health and evidence based supported employment services on those with serious and persistent mental illness. A key position on this project will be the Nurse Care Coordinator who will work with program participants and prescribers to implement systematic medication management and coordinate mental health and somatic care for all program participants.
The selected candidate will be a certified Psychiatric and Mental Health Nurse RN-BC. Prior experience working with prescribers, monitoring medication usage and results, resource coordination and psychiatric and somatic care documentation experience preferred.
